Biostimulators vs. Fillers: What's Actually the Difference?

I get asked for filler several times a week.

Usually the person asking has already made up their mind before they sit down. They've looked in the mirror, they've seen something online, they've decided the answer is volume. Sometimes they're right. But a good number of them are describing something else — skin that's gotten thinner, a face that reads tired rather than hollow, a jawline that used to have an edge and doesn't anymore. Those aren't always filler problems, and treating them with filler tends to produce the heavy, slightly-off look that everyone says they want to avoid.

Part of the confusion is on us, honestly. A lot of practices list Sculptra and Radiesse on their menus under "fillers." I understand why, it's simpler, and patients search for filler. But the products don't work the same way, and the difference changes almost everything about what your treatment plan looks like.

Filler adds volume

That's it. That's the mechanism.

Most fillers in use today are made of hyaluronic acid, a sugar molecule your body already produces, formulated into a gel of varying thickness. It gets injected into a particular tissue plane, it takes up space, and that space is occupied the moment the needle comes out. Lips. A deep fold. Structural support along the cheek or the chin.

Two consequences worth knowing. The first is that you see it immediately, swelling settles over a couple of weeks, but the change is there on day one. The second is that hyaluronic acid can be dissolved. There's an enzyme, hyaluronidase, that breaks it down. If placement isn't right, or someone decides a year later they'd rather not have it, we have a way out. That reversibility is a real safety feature and it's a large part of why HA is still what I reach for around the eyes and mouth.

What filler will not do is change the quality of the skin sitting on top of it.

Biostimulators don't fill anything

They provoke a response.

Placed in the tissue, these products set off a controlled, low-grade healing process. Fibroblasts (the cells that manufacture collagen) get recruited to the area and start producing. The product itself breaks down and clears over time. What's left behind is tissue you built yourself.

This is a slower and frankly more demanding proposition for the patient, and I'd rather people hear the downsides from me at the consult than discover them at week two.

Collagen remodeling takes months. Not days. If you are treated in March expecting to look different for a wedding in April, you are going to be disappointed, and you will be disappointed for reasons that have nothing to do with whether the treatment worked. These products are also planned across a series of sessions spaced weeks apart, because repeated stimulation is how the biology actually responds. That's a clinical course of treatment, not a package we've assembled to sell you more.

The upside is that the change arrives quietly. Nobody at Thanksgiving asks what you had done. It accumulates.

The other thing to understand is that a collagen response can't be dissolved. There's no enzyme for it. That doesn't make these products dangerous (I use them constantly) but it does mean the assessment beforehand carries more weight than it does with HA.

Sculptra and Radiesse aren't interchangeable

Both are biostimulators. They behave quite differently.

Sculptra is poly-L-lactic acid, and it's the purer collagen stimulator of the two. It adds essentially no volume up front. Everything you eventually see is your own tissue, which is why the expectation conversation matters more with Sculptra than with almost anything else I do. It gets reconstituted before treatment and injected across broader regions rather than into a single fold. When someone has diffuse facial volume loss and wants the change to show up gradually, this is usually where I go.

Radiesse is calcium hydroxylapatite — microspheres suspended in a gel carrier. That gives it a split personality. The carrier provides some immediate volume, which then resorbs over the following months while the microspheres do the collagen work. It's firmer and more structural, which makes it useful along the jawline. Diluted down, it also works well for skin quality in places that are otherwise difficult to treat: the neck, the décolleté, the backs of the hands, where volume was never the issue in the first place.

Two things I say out loud about Radiesse at every consult.

It is not dissolvable. There is no reversal agent for calcium hydroxylapatite. Once it's in, we manage the result rather than undo it, which is why I'm particular about where I place it and how much.

And I want your allergy history before we book, not on the day of treatment. Certain hypersensitivity histories change what I'm willing to do, and that's a conversation, not a checkbox.

Wrinkle relaxers are a separate thing entirely

This comes up in the same breath often enough that it's worth addressing.

Neuromodulators don't add volume and they don't build collagen. They temporarily reduce the muscle contraction that's creasing the skin — forehead lines, the elevens, crow's feet. Lines that appear when you move. If a line is being folded into your skin by muscle activity, no amount of filler is going to fix it, and I've seen plenty of faces where someone tried.

Most treatment plans end up combining categories, because most faces are showing more than one kind of change at once.

So which one do you need?

Depends on what's actually going on in your face, which means someone has to look at it.

I realize that's a frustrating thing to read at the end of an article. But two patients the same age with the same complaint routinely need different things, one a syringe of filler, the other a biostimulator series over several months, and often some combination sequenced deliberately rather than all at once.

What I'll commit to is that if the answer is a single syringe, I'll say so. And if the answer is that you don't need anything yet, I'll say that too. We've had that consult more than once.

Frequently asked questions

Is Sculptra a filler? No. Sculptra is a biostimulator made of poly-L-lactic acid. It prompts your body to produce its own collagen over time rather than adding volume directly, which is how a hyaluronic acid filler works.

Is Radiesse a filler? Radiesse is a biostimulator. Its gel carrier does provide some initial volume, but that carrier resorbs — the lasting result comes from collagen produced in response to the calcium hydroxylapatite microspheres.

How long before I see results from a biostimulator? Change is gradual and generally becomes noticeable over a period of months. Seeing very little early on is expected and isn't a sign the treatment failed.

Can biostimulators be dissolved if I don't like the result? No. Hyaluronic acid fillers can be dissolved with hyaluronidase. There is no equivalent for a collagen response, which is why careful assessment beforehand matters so much with these products.

Why are biostimulators done as a series? Collagen production responds to repeated, spaced stimulation. The series is how the product is designed to work clinically.

Can I combine biostimulators with fillers or wrinkle relaxers? Often, yes. The three categories address different problems, and most faces are showing more than one. Sequencing gets planned at consultation.
